Please refer to the CDAS service leaflet and complete all sections as fully as possible in order for this referral to be processed.  Any incomplete information may result in a delay to the referral process. When this form is completed, please send to the email addresses included at the end of this document.

Please read this carefully and use this temporary referral form during the COVID19 period.

Please be aware that we are not currently running face to face support groups for children and young people and are unable to offer face to face 1:1 appointments at this time. If you are referring to our usual groups – Helping Hands, CRUSH and Recovery Toolkit then please note that we are unable to confirm when these will be starting again due to the COVID19 restrictions and government guidelines.

You can use this form to refer to our usual services but please be aware that we may only be able to offer ‘keeping in touch’ support with families at this time until our usual groups and face to face support resumes.

We thank you for your understanding and patience and we hope to resume our usual support packages as soon as is safe to do so.

Please note 1:1 support is offered to those who cannot access group support; the wait for 1:1 support is slightly longer due to capacity. Please specify why the child/young person needs 1:1 support: The CDAS team may assess the child or young person to be better suited to a different service than initially referred for.

How did you hear about us?

Thank you for your referral. Once the team receive and process the referral the child or young person will be placed on a waiting list for a support. We may need to contact you to obtain any further information. We aim to offer the support as quickly as possible, while also ensuring the child or young person is offered the most appropriate support package for them. Please provide the family or young person with a CDAS service leaflet which contains WMWA contact details should they wish to contact us.

Please be aware that the CDAS team work term time only and should referrals be sent during the holidays they will be processed once the team returns to work.
